Application Form

The Occupational Physicians Scholarship Fund has been established to help alleviate the shortage of properly trained and skilled occupational physicians. OPSF will assist qualified individuals, capable of making a significant contribution in their field, to pursue a well-rounded residency education in occupational medicine. It is the goal of OPSF to provide financial support to residents in occupational medicine, in order that they become certified specialists able to practice in a broad range of settings.

The OPSF stipend support is structured to help defray costs of tuition, books, other academic expenses and a modest living allowance. Indirect costs to the institution the resident is attending are not covered under the scholarship.

Applicants to any occupational and environmental medicine residency-training program approved by the Accreditation Council for Graduate Medical Education (ACGME) within the United States are eligible to apply for scholarship funding.

Applicants are required to submit to OPSF the following:

  • A completed OPSF application downloaded from this web site
  • All undergraduate college transcripts
  • Records of previous medical and post-medical school performance
  • Copies of any publications that the applicant has authored
  • Three letters of recommendation, including letters from the directors of any previous training programs
  • A letter of support from the applicant's current program director, if accepted in an occupational medicine program

Written requests for applications should be sent to the OPSF Program Director, 25 Northwest Point Blvd., #700, Elk Grove Village, Illinois, 60007-1030.
